Dependent Eligibility Changes

A notice was sent March 31st, explaining that due to the changes regarding extended coverage for dependent children to age 26, it is necessary for each participant to enroll/reenroll their dependent children between the ages of 19 - 26 (children are no longer eligible as of their 26th birthday), including children currently covered as full-time students.

1	Transfer Pension Contribution Out (PDF) This form is used when you were working in our jurisdiction and you are a member of another Pension Plan. This form should be completed and sent to the Ohio Carpenters’ Pension Plan to have your Pension hours sent back to your home fund.	0.016886	01.15.2010
2	Transfer Pension Contribution In (PDF) This form is used when you are a member of the Ohio Carpenters’ Pension Plan and you were working outside of our jurisdiction. 3	Beneficiary Card (PDF) Use to designate a beneficiary for your Pension Plan. In the event of your death while participating in the plan, your designated beneficiary shall be entitled to receive a death benefit.	0.148737	01.15.2010
