Please remember...

Return all forms sent to you in a timely manner – health care claims are held until they are returned.  If you get married, divorced or have a baby, call the Fund office and let us know as certain documents are required for these events.

Appropriate documentation for such events is required.  You must notify the Plan if your 19 - 26 year old dependent becomes eligible for coverage.  If your spouse obtains coverage through their employer, the Fund office must be notified.  Also, remember to change your address if you move. 

Any payments made in error by the Hospitalization Plan because you did not inform us or other health care plan coverage and/or a divorce will be your responsibility for repayment.
